The Niagara Ontario Health Team continues to highlight to success of their ‘Let’s Go Home’ program.
Over the past 10 months 315 patients in the region have benefited from the program.
Director of Access and Flow, Lisa Hildebrand, says the goal is to take patients that are over 65 and no longer need hospital level services and return them to their home.
Those in the program can access services such as meals, homemaking, transportation, respite, and assistance navigating other health care options in the community.
The program was started with $500,000 from the Ministry of Health.
Last week an additional $125,000 was announced and that will allow for at least 50 more patients to return to their homes by the end of March.
